ABSTRACT

OBJECTIVE To prepar e and characterize danazol (DAZ)-sodium caseinate (SC)composite nanoparticles ,and to study the mechanism of preparing nanoparticles in “bottom-up”technology. METHODS SC was used as a stabilizer for regulating nanoparticles,so that DAZ-SC composite nanoparticles were prepared by anti-solvent precipitation method. The particle size , Zeta-potential,micro-morphology,stability,encapsulation efficiency ,drug loading and in vitro dissolution rate were characterized. Fluorescence spectra ,IR spectra ,FBRM and other methods were used to analyze the interaction mechanism between DAZ and SC. RESULTS The particle size of DAZ-SC composite nanoparticles was (223.7±12.5)nm,and the polydispersity index was 0.274± 0.012. Zeta-potential was -(17.81±1.63)mV(n=3). The stability of nano-suspension was good ,the solid properties of DAZ were greatly improved ,and the dissolution rate was significantly increased. SC was statically quenched under the action of DAZ and the secondary structures of SC were changed. The crystallization process of DAZ was controlled under the action of SC ,and the interaction between DAZ and SC was mainly hydrogen bond and van der Waals force. CONCLUSIONS In this study ,DAZ-SC composite nanoparticles are successfully prepared. In the “bottom-up”technology,the interaction between SC and DAZ caused by hydrogen bond and van der Waals force inhibits the growth and agglomeration of drug crystals .

ABSTRACT

Objective@#To evaluate the efficacy and tolerability of ruxolitinib combined with prednisone, thalidomide and danazol for treatment of in myelofibrosis (MF).@*Methods@#Patients of MF according to the WHO 2016 criteria, received ruxolitinib (RUX) combined with prednisone, thalidomide and danazol (PTD). The response, changes of blood counts and adverse events were evaluated.@*Results@#Six PMF and one post-ET MF patients were enrolled. Four patients presented JAK2V617F mutation, one CALR mutation, one MPL mutation, one triple-negative. Responses per IWG-MRT criteria were clinical improvement in 5 patients, stable disease in 2 ones, spleen response in 6 ones. All of 7 patients were symptomatic responses, four patients achieved at least 50% improvement from baseline on MPN-SAF TSS. Three patients initially treated with RUX alone, all of 3 patients experienced treatment-associated anemia and thrombocytopenia. Then these 3 patients received RUX combined with PTD, both hemoglobin and platelet increased significantly. Four patients initially treated with RUX combined with PTD. Increased levels of hemoglobin and platelet were seen in all of 7 patients received RUX combined with PTD with maximum increased hemoglobin of 30(18-54) g/L and maximum increased platelets of 116(13-369)×109/L, respectively from baseline. The treatment dose of RUX increased due to improved platelet count in 3 patients. The frequent non-hematologic adverse events grade 1-2 were constipation, abdominal distension, crura edema and increased ALT.@*Conclusions@#RUX combined with PTD for treatment of MF may modulate initial hematologic toxicity observed when RUX alone, and may increase response due to improved levels of hemoglobin or platelet.

ABSTRACT

Objective To develop an HPLC-MS/MS method with solid nuclear particle chromatographic column to determine 15 afrodyn drugs illegally added in sex-enhancing product. Methods The analysis was performed by an HPLC-MS/MS system, with Cortecs-C18 (150 mm × 4.6 mm, 2.7 μm) column. Multiple-reaction monitoring (MRM) was performed to identify and quantify phentolamine mesylate, noracetildenafil, acetildenafil, vardenafil, sildenafil, hydroxyhomosildenafil, homosildenafil, aminotadalafil, tadalafil, thioaildenafil, pseudovardenafil, norneosildenafil, methyltestosterone, stanozolol and danazol, which were extracted with methanol by ultrasonic. Results The linear calibration curves of 15 chemical components mentioned above were obtained at r2 ≥ 0.995 4 with better separation degree and wide linear range. The precision of the method was shown by RSD (n = 6) from six estimated values ranged from 0.9% to 3.3%. The recovery rates determined at three adding concentrations were from 88.3% to 109.5% and the RSD was from 0.9% to 5.2%. The range of limit of quantitation (LOQ) were from 0.015 μg/mL to 0.30 μg/mL and The RSD of inter-day and intra-day precision were from 0.9% to 4.2% and from 1.0% to 4.5%, respectively; Twenty batches of positive samples of 25 batches of sex-enhancing products were detected to be added with sildenafil and with an unqualified rate of 80%. Conclusion The method is specific and simple to detect that 15 afrodyn drugs were added illegally in sex-enhancing product. At the same time, the high positive detection rate should attract the attention of the department concerned, and the corresponding policies should be issued to guarantee the health and safety of the people.

Abstract: Background: Hereditary angioedema is a rare autosomal dominantly inherited immunodeficiency disorder characterized by potentially life-threatening angioedema attacks. Objective: We aimed to investigate the clinical and genetic features of a family with angioedema attacks. Methods: The medical history, clinical features and C1-INH gene mutation of a Turkish family were investigated and outcomes of long-term treatments were described. Results: Five members had experienced recurrent swellings on the face and extremities triggered by trauma. They were all misdiagnosed as familial Mediterranean fever (FMF) depending on frequent abdominal pain and were on colchicine therapy for a long time. They had low C4 and C1-INH protein concentrations and functions. A mutation (c.1247T>A) in C1-INH gene was detected. They were diagnosed as having hereditary angioedema with C1-INH deficiency (C1-INH hereditary angioedema) for the first time. Three of them benefited from danazol treatment without any significant adverse events and one received weekly C1 esterase replacement treatment instead of danazol since she had a medical history of thromboembolic stroke. Study limitations: Small sample size of participants. Conclusion: Patients with C1-INH hereditary angioedema may be misdiagnosed as having familial Mediterranean fever in regions where the disorder is endemic. Medical history, suspicion of hereditary angioedema and laboratory evaluations of patients and their family members lead the correct diagnoses of hereditary angioedema. Danazol and C1 replacement treatments provide significant reduction in hereditary angioedema attacks.

ABSTRACT

Objective To study the effect of low-dose cyclosporine combined with danazol on chronic aplastic anemia,and to analyze the influence on peripheral hemogram.MethodsRetrospectively analyzed the clinical data of 68 patients with chronic aplastic anemia treated from July 2014 to June 2016 in the second hospital of shandong university,according to their treatment methods were divided into control group and observation group,the control group was given conventional dose of cyclosporine combined with danazol,the observation group received low-dose of cyclosporine combined with danazol,four weeks for a course of treatment.After two courses of treatment,observe the curative effect of two groups,the difference of bone marrow,peripheral blood,immunoglobulin levels before and after treatment were compared in the two groups.Results The effective rate of the observation group was 96.97%,which was significantly higher than that of the control group 77.14%(F=6.563,P=0.028).After treatment,the bone marrow of the observation group was significantly improved compared with the control group(P<0.05); the levels of T-bet,GATA-3 and STAT4 of the observation group were lower than those of control group(P<0.05),the levels of WBC,Hb,PLT and RET were higher in the observation group than in the control group(P<0.05); there was no significant difference in serum immunoglobulin level between the two groups after treatment,there was no significant difference in the incidence of liver and renal function,gastrointestinal reaction,hairy and acne between the two groups.Conclusion Low-dose cyclosporine combined with danazol has better therapeutic effect in the treatment of chronic aplastic anemia,can significantly improve the hemogram and myelogram of patients,has good clinical value high security.

ABSTRACT

Introduction: Danazol is frequently used in the treatment of idiopathic thrombocytopenic purpura (ITP). Very occasionally are described in the elderly fatal respiratory complications related to this drug such as acute pulmonary fibrosis for which no treatment has proven to be effective. Presentation of Case: We describe a case of an old man with ITP treated with danazol who developed a pulmonary fibrosing disease not attributed to any other cause. Damaged lung reversed using an anti fibrosant therapy approved for other types of pulmonary fibrosing diseases. Discussion and Conclusion: Pulmonary fibrosing disease attributed to danazol is a condition refractory to systemic corticosteroids, but as shown in the present case, an anti fibrosant therapy has proven to be a useful alternative and could be recommended in the light of current knowledge.

Objective To observe the effects of Taohechengqitang combined with danazol on CA125 and gonadal hormone in the treatment of patients with endometriosis.Methods 77 cases from July 2014 to January 2015 in department of obstetrics and gynecology of Daqing Longnan hospital were selected and divided into observation group of 40 cases and control group of 37 cases according to double-blind randomized.The observation group were treated with Taohechengqitang joint danazol treatment, and the control group were treated with danazol alone.The estradiol ( E2 ) , progestogen (P), cancer antigen 125 (CA125), thromboxane B2 (TXB2), endometrial cysts size and dysmenorrheal score pre-and post-treatment between two groups were compared.Results The contents of E2 and P post-treatment in observation group were lower than those in control group (P<0.05).The serum TXB2 and CA125 levels post-treatment in observation group were lower than those in control group (P<0.05).The endometrial cysts size and dysmenorrheal score post-treatment in observation group were lower than those in control group (P<0.05).The total efficacy in observation group was higher than that in control group (P<0.05).Conclusion Taohechengqitang combined with Danazol could reduce the CA125, E2 and P levels, relieve symptoms of dysmenorrhea and reduce the cyst volume, whose efficacy is exact.

ABSTRACT

Objective: To establish an accurate method for the determination of nine chemical drugs (phentolamine mesylate, methyltestosterone, stanozolol, danazol, tadalafil, sildenafil citrate, aildenafil, vardenafil, and thioaildenafil) which were illegally added into the antifatigue health foods. Methods: The UPLC-MS/MS method was adopted. The samples were extracted with methanol by ultrasonic processing and separated on a Waters Acquity BEH-C18 (100 mm × 2.1 mm, 1.7 μm) column with 0.1% formic acid methanol (A) and 0.1% formic acid water (B) as the mobile phase by gradient elution (0-5 min, 50% A; 5-7 min, 50%-90% A; 7-9 min, 90%-100% A; 9-10 min, 100%-50% A) at a flow rate of 0.2 mL/min. The injection volume was 5 μL. The column temperature was 40 ℃. The positive-ion (ESI+) source and MRM mode were used to separate and quantitatively determine the chemicals. The obtained molecular ions, fragment ions, and retention time for MRM channels were used to identify the nine kinds of drugs by comparing with those of reference substances. The obtained peak areas were used to determine the accurate content of the nine chemicals in the antifatigue health foods. Results: A good resolution of the nine kinds of chemical drugs, including phentolamine mesylate, methyltestosterone, stanozolol, danazol, tadalafil, sildenafil citrate, aildenafil, vardenafil, and thioaildenafil, was obtained under this UPLC and MS/MS conditions. The limits of detection (LOD) and quantification (LOQ) were in the ranges of 0.1-0.3 ng/g and 0.3-0.9 ng/g. The standard addition recoveries were in the range of 88.4%-116.3%. There were 68 batches of antifatigue health foods, among which 41 batches were added with the chemicals with positive rate of 60.3%. The sildenafil citrate, tadalafil, aildenafil, and hioaildenafil were detected in samples. Conclusion: The method is simple, accurate, and has high sensitivity, which can be used for the qualitative and quantitative determination of illegally added chemical drugs in the antifatigue health foods.

[ABSTRACT]AIM:Toexploretheroleofkisspeptin/GPR54signalingpathwayinthepathogenesisofprecocious puberty based on female precocious puberty rat model induced by the single dose of danazol .METHODS:Female SD rats aged 3 d were randomly divided into normal group , vehicle group and model group .On the 5th day, the model rats were given a single subcutaneous injection of danazol with ethanol and ethylene glycol mixture .All rats were executed on 15 d, 25 d, 30 d, 35 d and 40 d, and the samples were collected to observe the sexual organ development .The levels of E2 , FSH and LH in peripheral blood were measured by ELISA .The Kiss-1 mRNA expression of Kiss-1, GPR54 and GnRH in hypothalamus was detected by real-time PCR.The kisspeptin expression in rat hypothalamus was observed by immunofluo-rescence .RESULTS:The time for puberty onset and sexual maturation in the model rats was significantly earlier than that in normal group and vehicle group .On days 25 and 30, the levels of peripheral sex hormones and uterine coefficients in model group were significantly higher than those in normal group and vehicle group .On days 25 and 30, the ovarian mor-phological development in the model rats was significantly earlier than that in normal group .On day 25, the mRNA expres-sion of hypothalamic Kiss-1 and GnRH, and kisspeptin expression of hypothalamic arcuate nucleus in the model rats signifi -cantly increased compared with normal group and vehicle group .On day 30, kisspeptin expression of hypothalamic arcuate nucleus in the model rats decreased compared with normal group and vehicle group .On day 35, the mRNA expression of Kiss-1 and GnRH in the model rats decreased compared with normal group and vehicle group .The mRNA expression of GPR54 had no obvious difference among all groups .CONCLUSION:The Kiss-1 mRNA and kisspeptin expression in the model rats with precocious puberty is significantly increased in the hypothalamus during onset of puberty , suggesting that kisspeptin is an initiating factor for precocious puberty .Kisspeptin/GPR54 signaling pathway may play an important role in the occurrence of precocious puberty .

ABSTRACT

We report on an angioedema patient with a genetic defect in complement 1 inhibitor, manifesting migraine-like episodes of headache, effective prophylaxis with Danazol, and triptan for a treatment of acute clinical episode. The patient was 44-yr-old Korean man with abdominal pain and headache, who was brought into the Emergency Department of Seoul National University Hospital, Seoul. He suffered from frequent attacks of migraine-like headache (3-7 per month), pulsating in nature associated with nausea. Severities were aggravated by activity and his headache had shown recent progression with abdominal pain. No remarkable findings were observed on radiologic examination, brain magnetic resonance images and intracranial and extracranial magnetic resonance angiography. Danazol 200 mg every other day was subsequently used. Following administration of Danazol, symptoms showed improvement and the patient was discharged. While taking Danazol, the migraine-like episodes appeared to be prevented for about 2 yr. At the eighth month, he suffered a moderate degree of migraine-like headache; however, administration of naratriptan 2.5 mg resolved his problem. A case of genetic defect of C1-INH deficiency presented with headache episodes, and was controlled by Danazol and triptan. It suggests that pathogenic mechanism of headache in hereditary angioedema may be mediated by the neurogenic inflammatory-like physiology of migraine.

ABSTRACT

O Angioedema Hereditário (AEH) é uma doença resultante de distúrbios nos sistemas complemento, da coagulação e calicreína-bradicinina. A doença manifesta-se por edema subcutâneo, dor abdominal e edema de laringe com morte por asfixia. Trauma, estresse e ciclo menstrual podem desencadear as crises. O AEH tipo I é descrito em 85% dos casos com níveis antigênicos e funcionais do inibidor da Cl esterase (C1-INH) reduzidos. No tipo II, o defeito é funcional com níveis de C1-INH normais. No tipo III, não existe alteração do C1-INH e associa-se a elevados níveis de estrogênio exógeno e/ou mutações no gene do fator XII da coagulação. Os níveis de C4 encontram-se reduzidos no HAE tipo I e II. A dosagem de C1q é utilizada para diferenciar o AEH dos casos adquiridos. Na profilaxia em longo prazo recomenda-se o uso de antifibrinoliticos ou andrógenos atenuados caso mais de uma crise grave ocorra ao mês e quando o tratamento para os ataques não forem eficazes ou disponíveis. Na profilaxia em curto prazo deve-se usar concentrados do C1-INH, não disponível no Brasil, substituído pelo plasma com eficácia limitada. Nas crises de AEH, o único medicamento disponível em nosso meio é o icatibanto, antagonista do receptor de bradicinina, administrado por via subcutânea. O ecalantide é um inibidor da calicreína usado nas crises também não disponível no Brasil. O AEH é uma doença subdiagnosticada que pode ser controlada evitando-se o óbito por asfixia. Novos tratamentos estão sendo disponibilizados que podem resultar numa melhor qualidade de vida dos pacientes.


Hereditary angioedema (HAE) is a disease caused by disturbs of complement, coagulation and kalikrein-bradikynin systems. The disease presents relapsing subcutaneous swelling, abdominal pain and laryngeal edema causing asphyxia. Trauma, stress and menses can precipitate the attacks. HAE Type I is described in 85% of the cases with reduced antigenic and functional levels of the C-1 esterase inhibitor (C1-INH). In Type II, the defect is functional and C1-INH levels are normal. In type III HAE, there is no impaired C1-INH but high doses of exogenous estrogens and/or mutations in Factor XII gene have been found. C4 levels are reduced in HAE Type I and II. Serum Clq is applied for differential diagnosis of acquired angioedema. Long term prophylaxis is recommended with antifibrinolytic agents or atenuated androgens whether there is more than one severe attack per month and the treatment for the attacks are nor efficacious or available. For short term prophylaxis, C1-INH concentrates should be used, however they are not available in Brazil and it is substituted by plasma with limited efficacy. During HAE attacks, the only drug available in Brasil is icatibant, a bradykinin antagonist receptor. Ecalantide is a kallikrein inhibitor to be used in attacks, not availabie in Brazil yet. Therefore, HAE is a misdiagnosed disease that may be controlled preventing the death due to asphyxia. New treatment options have been available that might result in a better quality of life within the patients.

Estudios clínicos y epidemiológicos sugieren que el danazol ha sido considerado como un factor de riesgo para desarrollar hipertensión. Para proporcionar información adicional acerca de este fenómeno, en este trabajo fue caracterizado el efecto inducido por el danazol y el hemisuccinato de danazol sobre la presión de perfusión y la resistencia vascular en corazón aislado de rata a flujo constante (modelo de Langendorff). Los resultados, mostraron que; 1) el hemisuccinato de danazol [10-9 M] incrementa la presión de perfusión en comparación con el danazol [10-9 M]; 2) los efectos del derivado de danazol [10-9 M - 10-4 M] sobre la presión de perfusión fueron inhibidos por flutamida [10-6 M]; 3) la nifedipina [10-6 M], bloqueó los efectos ejercidos por el hemisuccinato de danazol [10-9 M -10-4 M] sobre la presión de perfusión y 4) el efecto del derivado de danazol [10-9 M - 10-4 M] sobre la presión de perfusión en presencia del montelukast [10-6 M] fue inhibido significativamente (p=0,008). En conclusión, los efectos inducidos por el danazol y hemisuccinato de danazol sobre la presión de perfusión y la resistencia vascular podrían depender de su estructura química. Este fenómeno podría involucrar la interacción del receptor de andrógenos e indirectamente la activación de la síntesis de leucotrienos D4 (LTD4) y consecuentemente inducir variaciones en la presión de perfusión.


Epidemiological and clinical studies suggest that danazol has been considered a risk factor for hypertension development. In order to provide additional information about this phenomenon, the effect induced by both danazol and hemisuccinate of danazol on perfusion pressure and vascular resistance was characterized in isolated rat heart at constant flow (Langendorff model) and it was evaluated in this work.The results showed that; 1) hemisuccinate of danazol [10-9 M] increases perfusion pressure and vascular resistance in comparison with danazol [10-9 M]; 2) the effects of danazol-derivative [10-9 M - 10-4 M] on perfusion pressure were inhibited by flutamide [10-6 M]; 3) nifedipine [10-6 M] blockaded the effects exerted by hemisuccinate of danazol [10-9 M -10-4 M] on perfusion pressure; and 4) the effect of danazol-derivative [10-9 M - 10-4 M] on perfusion pressure in presence of montelukast [10-6 M] was significantly inhibited (p=0.008). In conclusion, the effects induced by both danazol and hemisuccinate of danazol on perfusion pressure and vascular resistance could depend on their chemical structure. This phenomenon could involve the interaction of androgene steroid-receptor and indirect activation of leukotriene D4 (LTD4) synthesis and consequently, induce variations in the perfusion pressure.

BACKGROUND: Most of adult patients with chronic immune thrombocytopenic purpura (ITP) that was refractory or relapsed to high-dose corticosteroid have been treated with splenectomy as a 2nd line treatment. However, these patients may have increased morbidity and mortality according to the operation and the increased risk of infection for a lifetime after splenectomy. Despite of the above risks, 30~40% of these patients can't maintain remission. Furthermore, the remission rate after splenectomy is relatively lower in patients with corticosteroid-refractory chronic ITP than that in those patients with corticosteroid-responsiveness. We studied whether danazol, an attenuated androgen, is useful or safe as 2nd line treatment for chronic ITP instead of splenectomy and which factors are associated with the response to danazol. METHODS: Among the patients with chronic ITP who failed corticosteroid therapy in our hospital, 28 patients who received danazol as the 2nd line treatment were analyzed retrospectively. A complete response was defined that the platelet count was increased to 150 x 10(3)/microL, and a partial response was defined that the platelet count was increased above 50 x 10(3)/microL or there was an increased platelet count of more than 20 x 10(3)/microL from the pre-treatment platelet count when the platelet count was above 50 x 10(3)/microL at the time of danazol therapy. RESULTS: The median age of patients was 44 years (range: 19~67) and the number of male patients was 9 (32.1%) and the number of females was 19 (67.9%). The starting daily doses of danazol were variable from 200 to 600mg, though most of the patients were treated with 400mg daily (18 cases, 64.3%). The median duration of danazol therapy was 201.5 days (range: 13~973) and the median duration from ITP diagnosis to danazol treatment was 56 days (range: 20~2,430). Among the accrued 28 patients, 22 patients showed a response to danazol (78.5%); there were 6 patients (21.4%) with a complete response and 16 patients (57.1%) with a partial response. The median duration from danazol treatment to response was 30 days (range: 0~180). The median response duration of danazol treatment was 330 days (95% CI: 182~478) by the Kaplan-Meiyer method. For the danazol-responsive patients, 9 patients (40.9%) remained in remission and 13 patients (59.1%) relapsed. Grade 3~4 toxicity was observed in two patients and three patients stopped danazol because of adverse effects. Hepatotoxicity was the most common toxicity. CONCLUSION: Our findings suggest that danazol is a beneficial, safe choice as the 2nd line treatment for patients with chronic ITP that was refractory or relapsed to corticosteroid.

ABSTRACT

The objective of this paper is to discuss the current guidelines for treatment of endometriosis, emphasis on the role of laparoscopic surgery and medical treatment. The accuracy of diagnosis of endometriosis without laparoscopy is very low, as a false negative rate of 19 % and a false positive rate of 44%, when a diagnosis was made pre laparoscopy, 81% had the diagnosis can confirmed on laparoscopy, while 19% did not have endometriosis. It is concluded that laparoscopy is required for evaluation and treatment of endometriosis. Medical therapy is effective in reducing progression of endometriosis score.

ABSTRACT

Idiopathic thrombocytopenic purpura (ITP) is an immunologic disorder that destroys platelets and megakaryocytes. Hemorrhagic complications are common in patients with ITP. However, thrombotic complications such as coronary artery disease and cerebral infarctions are infrequent. We report a patient with ITP who developed a cerebral infarction despite thrombocytopenia after long-term danazol therapy and was negative for the antiphospholipid antibody.

ABSTRACT

Endometriosis is relatively common gynecologic diseases, but pulmonary endometriosis is exceedingly rare among various types of external endometriosis. Patients with pleural pulmonary endometriosis present with chest pain, dyspnea, pneumothorax or pleural effusion in relation to menstruation. Patients with parenchymal pulmonary endometriosis present with hemoptysis at the time of menstruation, with or without lung lesions on their chest X-ray. The diagnosis of pulmonary endometriosis is usually made on the basis of the clinical history and the exclusion of other causes of recurrent hemoptysis including tuberculosis, bronchial carcinoid, pulmonary infarction, chronic bronchitis, congenital abnormalities and carcinoma. It can be treated by progesterone, GnRH agonist, Danazol, surgical treatment etc. Hereby we experienced 23 year-old multiparous woman with catamenial hemoptysis. The site of disease/was localized with bronchoscopy and chest CT scanning, and we treated her with Danazol. The literature associated with pulmonary endometriosis is briefly reviewed.

Objective To ob se rve the effects of danazol on hypothalamus-pituitary-gonad axises and expressi ons of hypothalamic GnRH and pituitary GnRH receptor mRNA in female rats. OBJECTIVE:To establish a method for the assaying of danazol in anti-hyperplasia of mammary glands cream.METHODS:The UV-spectrophometry was adopted to determine the content of dianozol with the wavelength at285nm.RESULTS:The dianzol was in linear relationship within the concentration range of5~25?g/ml(r=0.9999).The average recovery rate was99.2%(RSD=0.32%,n=5).CONCLUSION:The method is simple,rapid and accurate,which is suitable for the content determination and quality control of the preparation.
